The Soldier for Life: Transition Assistance Program (TAP) aims to deliver a world-class transition program for America’s Army that empowers members to make informed career decisions that will enhance lives, communities, and our nation.

TAP enables Soldiers to capitalize on Army experience and skills and make informed decisions using the skills and knowledge gained from this program. This decision may include reenlistment, attending college, seeking employment, attending additional training, going into business for themselves, or other options. These choices are made through informed decision making and set Soldiers up for success.

As part of the mandatory curriculum, Soldiers must complete the Preseparation Counseling that results in the creation of the Preseparation Counseling Checklist (DD Form 2648) NOT LATER THAN 12 months prior to separation; it is recommended that this is completed 15 months prior to separation or up to 24 months prior to retirement.

Additional mandatory classes include VA Benefits I and II, Financial Counseling, and the Department of Labor Employment Workshop. Furthermore, Soldiers must meet with a career counselor and establish an Individual Transition Plan (ITP), present a 12-month post transition budget along with a resume and complete two employment applications prior to separation. Successful completion of these requirements is annotated on the Individual Transition Plan Checklist (DD Form 2958).

Soldiers must complete three surveys in conjunction with the program, these surveys are:

  • Pre-transition Survey
  • Exit Survey
  • Post-Transition Survey

Soldier for Life - Transition Assistance Program

Online Enrollment Instructions


(**NOT MAC FRIENDLY**) The goal is to complete the pre-transition questionnaire.


(**NOT MAC FRIENDLY**) The goal is to complete form DD2648.

The Preseparation Counseling satisfies SFL-TAP enrollment requirements and is 1 hour and 45 minutes. The brief is completed by answering “Yes” and “No” questions regarding a Soldier’s transition. Progress can be saved and revisited later. Regardless of the answer, all Soldiers will receive the entire brief. The Soldier must complete the DD2648 to satisfy this requirement.

1. Go to
2. Click “LOG-IN” at the top right corner
3. Enter in your AKO User Name and Password
4. Register or Update Personal Information and click “accept”
5. Click “Complete Online”

NOTE: If a “white screen” appears while using a government computer click on the gear symbol in the right hand corner. Click on "compatibility view". Click "add" then "continue".

Once the Preseparation Counseling is completed, call the SFL-TAP center at 808-655-1028 to schedule the 60 minute initial counseling. Initial Counseling may also be completed remotely by calling the SFL-TAP 24 hour call center at 800-325-4715. IMPORTANT: You must complete additional requirements outlined at “Additional Requirements” Tab

3. EBENEFITS –create DS LOGON use CAC CARD for premium access
   Go to:
      Step 1: Click on REGISTER tab located at the top right side of the website
      Step 2: Follow the steps to create the account
      Step 3: PRINT out screen shot as proof of creating account
   This account will be your “go to website” for all your benefits from the VA such as accessing your Education Benefits, Applying for the Home Loan Certificate, Tracking your Disability claim should you have one and much more.

4. MyHealthEVet
   Go to:
      Step 1: Click on “register today” button on the right side of the website
      Step 2: Create Username and Password
   Creating this account will provide you with access to download your medical records as well as labs, appointments, prescriptions, and other medical records. (Do not print out medical records in bldg 750 office)

   Go to:
      Step 1: Complete Interest Profiler Survey
      Step 2: Browse through the categories from entry level careers to expert level careers
      Step 3: Please print the results page with your score of the assessment
      This is a great resource when researching the labor market and possibilities for types of employment.

6. VMET –Verification of Military Experience and Training (MOS JOB DESCRIPTION)
   Go to:
      Step 1: Click on LOG IN
      Step 2: Click OK
      Step 3: Use CAC LOGIN or can use DS Log On
      Step 4: Click “VMET” tab on the top of the page
      Step 5: Click on each bubble one at a time
      Step 6: Print VMET and cover letter documents

Go to:
      Step 1: Click OK on Consent Banner
      Step 2: LOG IN with CAC
      Step 3: Select TRANSCRIPTS at the top left
      Step 4: Select 1. Transcripts
      Step 5: PRINT Document
